Things you should know about your first visit...
Be aware that your initial visit can take at least an hour. We ask that you arrive 15 minutes before your appointment, at which time we will ask that you fill out our new patient form.
- Bring your insurance cards with you. You are responsible for your co-pay and/or co-insurance at the time of your visit. Payment may be made by cash, credit or check.
- Be aware that your initial visit with us is an evaluation only. We are generally not able to perform procedures on that date.
- During your evaluation you will be asked to undress from the waist down. You may wish to dress with this in mind.
- You are welcome to bring a friend or family member with you.
- Feel free to contact us with questions. We pride ourselves in being knowledgeable and attentive to your needs and requests.
Expect to receive lots of educational materials about your problems. We want you to understand what is going on and actually participate in your care.
